PTOSIS : (Abnormal Eyelid Position)

Drooping of upper eyelid can be present from birth or occur at a later age. The cause of drooping can be a weakness of the muscle or affection of the nerve supply to muscle.
A weakness of the muscle can be corrected by surgery.


Treatment for Ptosis :
a) FRONTALIS SLING SURGERY

If the eyelid muscle is very weak, the forehead muscle (Frontalis) is utilized to lift the eyelid. It is connected to the eyelid by a sling which is usually made of medical grade silicone.
Incomplete closure of the eye mostly during sleep is expected post operatively

b) LEVATOR PALPEBRAE SUPERIORIS

This is the muscle which is most important for lifting the upper eyelid. If muscle is weak, it is strengthened. If the muscle is displaced from its normal position, it is reattached. A second sitting for readjustment might be required in some patients to correct an overcorrection or under correction.

TURNING OF EYELID

a) INWARD TURNING OF EYELID ( ENTROPION )

If the eyelid margin is turned towards the eye, eyelashes rub against the cornea and cause severe distress.
Temporary relief is obtained from lubricants, taping of eyelid and Injection Botox. Surgery is done to bring the eyelid to its normal position and stop rubbing of the eyelashes.


b) OUTWARD TURNING OF THE EYELID (ECTROPION)

With age, the muscle around the eye become loose and it may result in the eyelid falling away from the eye. This may also occur because of scarring. An outward turned eyelid exposes the outer layer of the eye(conjunctiva) which causes redness and irritation. Also increased watering from the eye as a normal position of lid is of utmost importance in drainage of tears.
Surgery is done to correct the loose eyelid or correct the scarring.

A) WATERING IN ADULTS:

It can be because of abnormal position of eyelid or due to tear duct blockage. If blockage is suspected the doctor will perform a procedure called syringing wherein a fine tube is placed in the tear drainage system to check for blockage . It is performed in the clinic. A blocked tear duct can be treated with surgery.
The operation is called a Dacryocystorhinostomy(DCR) . A stent maybe placed during the DCR surgery which is removed at 3-6 months.


B) WATERING IN INFANTS:

In an infant it is mostly due to blocked tear duct. Proper technique of massage is advised which helps in clearing the blockage. Consultation with the doctor for correct technique of massage improves outcome.
If massage does not help your child, the doctor will advise you regarding Probing . A fine probe is passed through the tear duct to unblock it.
